Chaperones Clearance

Parents who would to volunteer/chaperone on an ongoing basis, must complete the DCPS Clearance process beforehand. Upon receipt, clearance letters should be emailed to tanisha.montgomery@k12.dc.gov. We'd love for as many caregivers as possible to go through this process and join us on our field trips this coming year.

SY26-27 Athletic Participation Packets
(4th and 5th Grade Only)

 

The DCIAA participation packets for SY26-27 are available on the DCPS Athletics Department website. The SeamlessDocs links have been updated as well. Families are strongly encouraged to submit this information over the summer to ensure students are medically cleared prior to the beginning of the sports season. An updated health form is also required.

Please note: submission of forms do not guarantee placement on the interested team. Your child may still be required to tryout for the interested team.

Student Health Forms & Medication Needs

All students are required to submit an updated Universal Health Certificate and Oral Health Assessment by the first day of school. Forms can be uploaded directly into student's ASPEN account (preferred) at aspen.dcps.dc.gov or emailed to shelia.roy@k12.dc.gov or 

roslyn.shields@k12.dc.gov.

Alternatively, hard copies can be submitted to your school's main office. A completed and updated Health Certificate is also required for 4th and 5th grade students to participate in DCIAA sports.

 

Whenever possible, administer medications at home. If your child needs access to medications or medical treatment during school hours, please complete the following steps:

  1. Have your medical provider complete the appropriate forms – Medication and Treatment Authorization Forms, the Asthma Action Plan and the Action Plan for Anaphylaxis. These forms are available from your school’s main office and online: https://dcps.dc.gov/node/1003562

  2. Drop off the medicine and required paperwork to the school nurse. In the nurse’s absence, they can be turned into Ms. Carraway at Watkins and Ms. Shields at Peabody. Medications will not be accepted without the appropriate paperwork.

  3. Communicate your child’s medical needs to their homeroom teacher. Please note that your child will not be able to receive medication during the school day (including on field trips) until steps 1 and 2 have been completed. IMPORTANT: If your child is able to self administer, please ensure that your medical provider identifies that on the form!
